During a periodic review of agent license and appointment information, Anthem has found that some agents’ information does not match their state Departments of Insurance (DOI) records. Anthem needs to change their information to match the DOI to ensure their records and commission payments are up to date and compliant.
Anthem will begin making these updates on March 14, 2025.
These updates may change some agents’ license or appointment termination dates, and as a result, impacted agents could receive an automated termination letter. Agents licensed or appointed in multiple states may receive more than one letter.
These termination letters are informational only. No action is required.
If you would like to obtain a license and/or appointment, follow these steps on or after March 20, 2025:
Anthem Blue Cross CA and NY (upstate)
Anthem Blue Cross Blue Shield CT, GA, NH, NY (downstate), and VA

- If not already registered -Select register
- You will use your personal tax identification number.
- Once registered log back in with the credentials you have created.
- On the top right-hand corner there is a blue radio dial if you hover or click on it a menu will drop down
- Click “ACCOUNT DETAIL”
- On the next page select from the dashboard “APPOINTMENTS”
- The page will display SELF-INITIATED appointments and on the bottom of the page there is an orange box that reads “Start Appointment” Click there.
- You will provide information based on your own personal tax id number /ss# , then you will come to a section that will ask if you are a principal you either mark yes or no
- You will be asked for the type of appointment “DIRECT APPOINTMENT “ if appointing is through agent only , “Agency” if appointing the agency, “Sub-Agent if being appointed under an agency but are not the agency’s Principal .
- You will select the state you wish to be appointed in and the line of business Small Group , Individual, etc. Please verify ALL lines of business that you will be signing are checked otherwise appointment for those products not checked may require an additional appointment submission.
- Proceed to completing all the documents, make sure you sign and date each form. The final item is payment. Once payment is complete you will hit submit appointment
