2025 Notice of Health Plan Changes for your Sutter Health Plan clients:

• Individual and Family Plan Notice of Health Plan Changes
• Small Group Notice of Health Plan Changes
• Large Group Notice of Health Plan Changes

Your clients that already received their renewal packets — including all Individual and Family Plan subscribers; small groups with effective dates through March 1, 2025; and large groups with effective dates through May 1, 2025 — will receive their respective updated 2025 Notice of Health Plan Changes under separate cover.

All other group employer clients will receive the updated 2025 Notice of Health Plan Changes in their upcoming renewal packets.
